The Memory Care Alliance and Comfort Keepers of Santa Fe
presents a community conversation about end-of-life decisions and
making your wishes known.

See the hour-long moving documentary, followed by a panel discussion with
Dr. Karin Tron, Hospice Medical Director, Jan Jahner, RN, Palliative Care, CSVH,
Leslie Van Pelt, Comfort Keepers, and Kristin St. Clair, RN at CSVH Holistic
Wellness Center. Join the conversation and learn how to take steps to identify and
communicate your wishes about end of life goals.
John Ruybalid, Santa Fe’s reverse mortgage specialist, will Emcee this event.
Refreshments provided by Comfort Keepers, Aspen Ridge, and Kingston.
