Coming Home Connection provides all levels of non-medical in-home care, up to a 24/7 basis, including: companionship, meal prep and grocery shopping, light housekeeping, overnight supervision, showering, bed baths, toileting, transfers and support with walking and driving for errands. We offer a specialized end-of-life care service. Our unique non-profit models offers all the benefits of a home care agency, without the agency care fee. We train and background check all our caregivers.

Presbyterian Home Health and Hospice
Presbyterian Home Healthcare and Hospice of Northern New Mexico provides services for patients who either need, or are more comfortable, receiving care at their home. These services include home healthcare and hospice.
Our home healthcare program is designed for patients who need consistent medical care, but not the 24-hour monitoring of a hospital or nursing home. While many of our patients are seniors, home healthcare can also benefit people of all ages who are recovering from surgery or have experienced a hospitalization for severe illness.
Our hospice care program provides compassionate care with physical, emotional and spiritual support to patients and their families when curative measures are no longer the best option. Hospice services can often start much sooner than expected and are not limited to the final days of life, so please call us to discuss options.
Professional Case Management
Professional Case Management is a free in-home care provider for clients whose service to our country included the following: Employed at either Sandia Laboratory or Los Alamos National Laboratory or workers who were either miners, millers, or haulers. To qualify for the program, you must have a qualifying medical diagnosis and have a qualifying work history.

We have supported, counseled, and cried with spouses, children, and other loved ones for years before they ultimately made the decision to place their loved ones in our homes. We have written a book, Mindful Dementia Care, documenting our story and experience.
Upon reflecting on our experience and the needs we have met through the years, Vista leadership recognized that caregivers of elders with dementia and Alzheimer’s have yet another unmet need: to keep loved ones at home for as long as possible with the assistance of our professional staff. We also know well the physical and emotional toll caring for someone with dementia can have on caregivers, especially in the early stages of the disease, when professional, specialized care seems beyond reach. There is no one that can do this better than us. And there is no one better to help reduce the risk of transfer trauma when staying at home is no longer possible.
